Personalized support through professional evaluation and counseling are the foundation of our practice. The comprehensive nature of our work begins with assessment of the senior's cognitive, memory and emotional functioning, and of the family's needs. We gain first-hand understanding of your situation and then provide living placement advice and coordination, on-site assessment, disposition of belongings, next location space planning and set-up.
Psychological Services are provided by a licensed clinical psychologist and begin with preliminary assessment of the senior's cognitive abilities, memory and emotional health, as indicated. The family's dynamics, needs and goals are ascertained through a detailed questionnaire and interview. Supportive counseling then allows the senior and family to successfully move through the decision-making process and transition.
Relocation Advice and Support Services are provided by SageHeart Advisors staff familiar with senior living options. The needs and desires of the senior and family members, level of functioning, geography, finances and other factors are considered and personalized advice follows. The additional SageHeart Advisors' services support the transition once the decisions are made.
